What paperwork do I need?

In order to receive a fair evaluation, you must be ready to supply as much recent medical documentation as possible. You will want to gather as many files, x-rays, prescriptions and proof of your health issues as possible. This documentation will assist the doctors in making their evaluation and deciding wether or not medical cannabis is right for you.

You will also need to fill out a complete medical history package and intake forms.These forms can be downloaded here, printed out and taken with you to your evaluation. Please complete pages 1-7 and 9-11, page 8 is reserved for Doctors observations. In order to expedite your raluation have the forms completed prior to arrival at your appointment.There is no need to share your medical history with us, we respect patients privacy very seriously.
